Agreements 

By registering to participate in the New England Epilepsy Convention and all of the events associated with it including, without limitation, the meetings, presentations, discussions, meals, social events, the exhibit hall, the NEEC Formal, and the Blue Skies Research Challenge (collectively, the “Convention Events”), I confirm that the information I provide in this registration form is true and complete. I agree to comply with all Convention Events policies, venue rules, and the code of conduct below. I understand that my registration may be denied or canceled for unsafe, unlawful, or disruptive conduct. 

Code of conduct 

For all Convention Events, I agree to treat all participants, speakers, staff, volunteers, vendors, and venue personnel with respect. Harassment, discrimination, intimidation, stalking, threats, disruptive behavior, and unauthorized recording of private conversations are prohibited. Epilepsy Foundation New England, Inc. (“EFNE”) may remove any person from the event for conduct that is unsafe, unlawful, or materially disruptive. 

Medical acknowledgment 

I understand and acknowledge that EFNE will not be providing any medical care, nursing care, or emergency medical services at the Convention Events. I am responsible for my own health needs, medications, and insurance coverage. Any first aid or emergency assistance, if available, is limited and does not create an obligation for EFNE to provide medical services. 

Dietary and allergy disclaimer 

I understand that food or beverages served at the Convention Events may contain allergens or may have been prepared in shared facilities. While EFNE will make reasonable efforts to account for the allergies and dietary needs I identify in advance, EFNE cannot guarantee that any food is allergen-free or safe for every dietary need. By signing this agreement, I acknowledge that I am responsible for my own dietary decisions and emergency preparedness. 

Alcohol acknowledgement 

I understand that alcoholic beverages may be available at certain Convention Events. If I choose to consume alcohol, I do so voluntarily and accept responsibility for my own decisions, conduct, transportation, and compliance with applicable law and event policies. 

I agree that I will not operate a motor vehicle, bicycle, scooter, or other vehicle while impaired or otherwise unable to do so safely. I will make responsible transportation arrangements, including use of a designated driver, rideshare, taxi, public transportation, or other safe option if I consume alcohol. 

I understand that alcoholic beverages will be served only to individuals who are at least 21 years old and who present valid government-issued identification upon request. I will not attempt to obtain, possess, purchase, provide, or consume alcohol in violation of law or event policy. 

I agree to comply immediately with directions from EFNE staff, event staff, venue staff, security personnel, and alcohol-service personnel. Refusal to comply may result in removal from the event without refund, where permitted by the registration policy. 

Accessibility notice 

EFNE is committed to hosting an accessible Convention Events and will make reasonable efforts to address accessibility requests made known to us in advance of the event. I understand that accommodation depends on feasibility, venue limitations, timing, available resources, among other factors. I agree to submit requests as early as possible so they can be reviewed in advance.  

Liability Waiver 

Release of Liability and Assumption of Risk 

In consideration of being permitted to attend or participate in the Convention Events, I, for myself and on behalf of my heirs, executors, administrators, assigns, and personal representatives, knowingly and voluntarily release, waive, discharge, and hold harmless EFNE, its officers, directors, trustees, employees, agents, volunteers, sponsors, contractors, the venue owner, and affiliated entities from any and all claims, demands, actions, liabilities, damages, losses, or expenses of any kind arising out of or relating to my attendance or participation in the Convention Events, except to the extent caused by the gross negligence or willful misconduct of EFNE. 

I understand that attendance at any or all Convention Events may involve risks including slips, trips, falls, crowding, illness exposure, transportation issues, equipment use, and other unforeseen hazards. I knowingly and voluntarily assume all risks associated with attendance and participation, whether occurring before, during, or after the event. 

I understand that this is a legal release and that I am giving up the right to bring certain claims against EFNE and the other released parties to the fullest extent permitted by Massachusetts law. 

I also understand that the event will not be staffed with a nurse or other medical team and that I will be responsible for my own medical care throughout the event. 

Photo, Name, and Likeness Release 

Consent for Promotional Use of Name and Images 

I grant EFNE and its authorized agents the irrevocable right to photograph, record, film, livestream, quote, reproduce, edit, publish, display, and distribute my name, image, likeness, voice, and statements in any medium now known or later developed, for lawful promotional, educational, archival, fundraising, and publicity purposes, without compensation or further approval. 

I understand that these materials may be used in websites, social media, newsletters, brochures, press releases, videos, and other publications. I waive any right to inspect or approve the finished materials and release all claims arising from authorized use of my name, image, voice, or likeness, except where prohibited by law.  

ATTENDEES, SPEAKERS, & VENDORS
By participating you agree that you are 18 years or older, have read and understand this registration packet, and agree to the terms and conditions of the Liability Waiver, the Photo, Name, and Likeness Release, the code of conduct, and all other aspects of this Agreement. 

PARENT OR LEGAL GUARDIAN 
(if registrant is under 18 years old or under the care of a guardian) 

By allowing participation, you agree that you are the parent or legal guardian of the minor named, and consent to the minor’s attendance at the Convention Events. You agree to the Liability Waiver, the Photo, Name, and Likeness Release, the code of conduct, and all other terms in this packet on behalf of the minor. 

You understand that a parent or guardian should sign photo-release materials for minors and that the organizer may require parental consent for a minor to attend or participate.
